In order to see all forms required for your student-athlete/participant, you will need to make sure you have chosen an activity or sport for the year of registration. Athletic physicals are valid for 2 years (starting on the date signed by physician) and are required to be completed on the new, revised athletic physical form (will have Rev. 2023 in the corner and is the form linked within the registration form sets). This form is available within FinalForms registration, on the OSAA website, and the EP athletics & activities website, eaglepointeagles.com, in both English and Spanish.
